بطبيعة القطط في حب الاستكشاف و التغيرات السلوكية اللي بتحصل مع تقدم عمرهم، فيه سلوك مشترك و منتشر بين كل القطط و هو السقوط من اماكن عالية!

احياناً بيكون بسبب حب الاستكشاف او في القطط الذكور عند ارتفاع مستويات هرمونات معينة عند مرحلة البلوغ و طلب الزواج.

للأسف حب الإستكشاف ده ممكن يؤدي للسقوط من ادوار مرتفعة زي حالتنا النهارده و عواقبه وخيمة من جروح بسيطة الي اصابات عميقة!

شوجر، بطل قصتنا النهارده؛ بشكل مفاجئ و هو بيلعب في البلكونة وقع من الدور التالت علي ارتفاع اكبر من 100 متر!

علي الفور عائلة شوجر توجهت لقسم الطوارئ في مستشفي د. بوز للحيوانات الأليفة، شوجر وصلنا المستشفي بأعراض عدم اتزان و شلل في الأرجل الخلفية و آلام شديدة!

تم التعامل معاه بالفحوصات و بأدوية الطوارئ اللازمة، الفحوصات اللي قام بيها د. شيماء أيمن وضحت انه في كسر في العمود الفقري نتيجة للإصابة و السقوط من مرتفع بإضافة إلي انفجار في حالب الكلية!

تم التعامل مع الحالة بالتدخل المناسب في الوقت المناسب لحد لما استقرت حالته الحمدلله.

Cats can suffer serious injuries when falling from buildings, balconies, or windows. These accidents can lead to a variety of physical traumas, including fractures, internal organs damage, thoracic injuries, head trauma, appropriate emergency, detailed exam and veterinary care is crucial in improving survival rates and long-term recovery for feline patients.

Case History

Sugar a 2 y old DSH cat represented with history of falling from high floor from balcony, and observed uncoordinated, paraplegic, dull, severe pain with 4th pain score.

شوجر وصل قسم الطوارئ في المستشفي في حالة صعبة جداً من عدم الإتزان و فقدان القدرة علي استخدام النصف السفلي من الجسم، كله ده مصحوب بآلام شديدة جداً من أثر السقوط!

Physical examination

Revealed tachyarrhythmia, hyperventilation, distended urinary bladder with abdominal palpation with bladder expression it could by evacuated and examine the musculoskeletal system showed instability in the spine.

دكتور شيماء بدأت الفحص بسرعة لإنقاذ شوجر، و اتضح ارتفاع في العلامات الحيوية عنده من زيادة بضربات القلب و زيادة معدل التنفس باللإضافة إلي تخلخل محسوس في العمود الفقري

Neurological examination

Preforming full neurological exam of the cranial and peripheral reflexes revealed LMN injury.

بالفحص العصبي في حالة شوج كان معقد جداً وضحلنا وجود مشكلة بشكل مبدئي في العمود الفقري في منطقة الفقرات القطنية، اثرت علي الامداد العصبي للأطراف الخلفية و بعض الأعضاء.

Laboratory findings

CBC showed decreased red blood cells count and hemoglobin concentration revealing microcytic-hypochromic anemia and thrombocytopenia, consequence with leukocytosis.
Serum chemistry showed hyperglycemia, azotemia which is high blood urea nitrogen and high creatinine concentrations in blood in addition to electrolyte disturbance appear as high phosphorus.

الاختبارات المعملية و تحديداً صورة الدم الكاملة وضحت لينا وجود انيميا شديدة عند شوجر مع ارتفاع في كرات الدم البيضاء، انتقالاً لوظائف الكلي اللي اضاءت مفتاح جديد في لغز شوجر و هو ارتفاع نسبة المخلفات النيتروجينية في الدم مما اشار لوجود إصابة بالكلي و وجهنا لعمل تصوير بالموجات الفوق سمعية علي البطن!

Diagnostic imaging

Ultrasonography: A-FAST and T-FAST scans preformed and anechoic fluid observed in the cysto-colic window, uro-abdomen confirmed with sampling the anechoic fluid.

Radiography: Intestines with mixed opacities indicative of distention with ingesta and gases, Spinal fracture observed with distended urinary bladder and opacity in the abdomen.

Revealing: Spinal fracture, Uro-abdomin, Constipation.

بعد القيام بالتصوير بالموجات فوق سمعية بالإضافة إلي التصوير بالأشعة السينية اتضح و تأكد وجود: كسر بالعمودد الفقري، تجمع غير طبيعي للبول في البطن.. و ده خلي ملف الحالة يتحول فوراً لقسم جراحة الطوارئ!

Emergency surgical exploratory laparotomy

🧩Upon exploratory laparotomy to identify the cause of uroabdomen which was thought to be *ruptured bladder* it was due to ruptured ureter consequenced to the trauma.

🧩The traumatized Ruptured Ureter led to persistent urine leakage into the abdomen which was due to ruptured ureter dealt with emergency nephrectomy.

🧩Accumulation of wastes in the blood due to innervation injury to the bladder also desvasting uroabdomen led to azotemic status, metabolic acidosis and electrolyte disturbances, dealt to the following postoperative treatment plan.

في جراحة الطوارئ الاستكشافية فريق الجراحة توصل الي سبب وجود البول في البطن و هو انفجار في حالب الكلية نتيجة للحادثة اللي حصلت لشوجر!

تم التعامل بحرفية من فريق الجراحة و توصلوا لأفضل حل ممكن و للأسف كان إستئصال الكلية نتيجة للتهتك الشديد اللي حصل في الحالب و كان يصعب إصلاحه و لحماية البطن من تجمع اي بول فيها تاني.

Treatment plan

Vitamin B complex

To support the neurological injury.

Kidney support

Essential amino acids as  L-Carnitine, L-Arginine, and Glutamine, promote the renal function and filtration process

Phosphate binder

To trap the elevated cation in blood and controlling the hyperphosphatemia

Furosemide

As a diuretic to flush the blood from the elevated nitrogenous metabolism byproducts and resolve the azotemic status

Lactulose and rectal enemas

Helping the the first week in the constipation state resulted from colonic inertia due to the neurological injury of the spinal cord.

Dexamethasone

As anti-inflammatory suppressing the leukocytosis state

بطلنا شوجر كمل معانا بعد العملية و المراقبة في وحدة الرعاية، في خطة العلاج كان عندنا مجموعة من الفيتامينات و منشطات الكلي مع المحاليل الوريدية و مدرات البول، بالإضافة إلي مضادات الإلتهاب.
